When reporting a loss, please provide the following information:
Date of Loss
Policy Number
Name and contact information of Insured
Loss Location
Loss Description
Names and contact information of all other parties/witnesses involved
Name and relationship to insured of person reporting loss
Documents related to loss, i.e. police report and damage estimate and attorney letter of representation.
Any person who knowingly and with intent to defraud any insurance company or other person files a statement of claim containing any materially false information, or conceals for the purpose of misleading information, concerning any fact, material thereto, commits a fraudulent insurance act which is a crime, subject to criminal prosecution and civil penalties.
